Mass In Memorum Card Request and Offering

If you wish to request a Mass in Memorum Card, we ask you to 1) complete the form below and 2) submit the offering via Give Central (a payment link to Give Central will be displayed after the form below is submitted).  Mass In Memorum Cards are $10.00. 

Please indicate your name, phone number, email address, Mass Intention, and recipient information. 

After we receive the Mass In Memorum Card Request and offering confirmation we will prepare and mail the Mass In Memorum Card.
